Step 2: How to Interpret the Mentzer Index

1. Mentzer Index < 13:

  • Suggests thalassemia
  • Patients with thalassemia often have a normal or high RBC count despite having small red blood cells (low MCV).

2. Mentzer Index > 13:

  • Suggests iron deficiency anemia.
  • In iron deficiency, both MCV and RBC count are reduced due to a lack of iron
  • needed for red blood cell production.

Step 3: What About Vitamin B Deficiency?

Vitamin B deficiency impacts MCV differently: MCV > 100 fL (macrocytic anemia):

Suggests a deficiency in vitamin B12 or folate.
